The 12-gauge rounds filled with metallic BB pellets are highly effective at point blank range, although the BB's lose their velocity after traveling a short distance and become harmless after awhile. Despite its limited ammo, it can obliterate hordes of infantry with a single shot and even destroy vehicles easily with just a few. The upgraded version has more range, at the cost of even less ammo.

- Infantry units inflicted upon by Shotgun are knocked back for a great distance.
- In Metal Slug 5, when the Shotgun is used while the player is sliding, the player's control of the character is delayed for a longer time compared to other special weapons.
